Individual Membership
$65 per month.
Anyone over 18.
This is your primary account.
Add Dependents (Optional)
$35 per month per dependent.
Minor Membership (17 and under, and must be attached to their parent or guardian individual membership)

Questions
Signing up for an individual membership is easy!
1) Fill out the 'Become a Member' form
2) A Benefits Coordinator will send you an email to create a portal login and complete the sign-up form
3) We will review your submission, and upon approval, send you a membership agreement to sign and get your ACH payments set up.
4) That's it! Your membership will be active on either the 1st or 15th of the month ( this will be provided to you with enrollment.
No. Community Care Clinic membership is available to everyone, including individuals and families who are uninsured, underinsured, or looking for a more affordable and personalized healthcare experience.
YMHCCC membership is not available to to beneficiaries of Medicare, Medicaid, CHIP, TriCare, or any other state or federal government healthcare program.
A primary care doctor focuses on your individual healthcare needs, such as routine checkups, preventive care, and managing chronic conditions. A YMHCCC provides access to these primary care services while also offering additional resources designed to improve the health and well-being of the broader community.
YMHCCC combines personalized primary care with wellness programs, health education, and exclusive member benefits—creating a healthcare experience that is accessible, affordable, and centered around the needs of individuals and families.
YMHCCC locations offer convenient scheduling options. You can walk-in, or schedule an in-person or Telehealth appointment. Contact your nearest participating clinic or review your membership details to learn more about appointment availability.
Individual membership (18 years and older) is $65 per month. Each additional dependent is $35 per month. Dependent membership must be attached to parent or legal guardian membership.
